Our Disease Management programs empower individuals, in collaboration
with physicians and other health care professionals, to effectively manage
their conditions and associated risk factors and co-morbidities. Our programs
are fully integrated with Personal Health Support clinical programs, ensuring
the coordination of clinical resources across the continuum of wellness,
health and medical decision support and Disease Management. We provide
a whole-person approach that includes behavioral health, co-morbidity
and prescription drug management. We help reduce variation in clinical
quality and cost through improved patient self management, and through
guidance to quality and efficiency designated physicians, Centers of Excellence networks
and clinical programs, and UnitedHealthcare® networks where
appropriate. We provide a complete view of the employer population and
individual member data from a shared technology platform across programs
with a Longitudinal Personal Record and through an integrated predictive
model and consolidated reporting.
Asthma Disease Management Program
Asthma is a prevalent, and growing, costly health problem for Americans.
The Asthma Disease Management program helps reduce unnecessary hospitalizations
and health care costs, missed days of work and seeks to improve the
individual’s quality of life. The program provides information, comprehensive
assessments, nurse coaching, interventions and other resources to help
participants:
- Avoid triggers that induce or aggravate asthma attacks
- Reduce or eliminate risk factors, such as smoking
- Manage their condition through adherence to medication, peak flow
meter use, and other physician treatment plans
- Monitor their asthma to recognize attack warning signs and take appropriate
actions
Cancer Support Program
Cancer Support Program is a comprehensive, integrated single source to
help manage cancer-related costs and to help improve the quality of care
and quality of life for your employees and their dependents. The program
is delivered through our team of specialized cancer nurses, who coordinate
care, help prevent and manage symptoms and side effects, collaborate with
physicians and other nurses and help patients navigate the complex health
care system. They will have access to cancer Centers of Excellence network
and UnitedHealth Premium® designated physicians .
Chronic Obstructive Pulmonary Disease
Chronic Obstructive Pulmonary Disease is
a lung disease in which the tubes that carry air in and out of the lungs
are partly obstructed, making it difficult to get air in and out. Most
people with this disease have both chronic bronchitis and emphysema.
Cigarette smoking is the most common cause of this disease, but those
who breathe in other kinds of lung irritants, like pollution, dust,
or chemicals, over a long period of time may also develop this disease.
There is no cure - the damage cannot be reversed, but there are treatments
that can help the person feel better and slow the damage. Our program
provides education, self management skills and can explain treatment
options that may be available to an individual, based upon the recommendation
of their health care provider to:
- Relieve symptoms with no or minimal side effects
- Slow the progress of the disease
- Improve exercise tolerance (the ability to stay active)
- Prevent and treat complications and sudden onset of problems
- Improve overall health
Coronary Artery Disease
Over time, Coronary Artery Disease can weaken the heart muscle and contribute
to Heart
failure (heart fails to pump blood properly) or Arrhythmias (a-RITH-me-as)
- changes in the normal beating rhythm of the heart. Some can be quite
serious. Our Coronary Artery Disease program is designed to help reduce
unnecessary hospitalizations and health care costs, and improve quality
of life. Through sophisticated stratification, members are targeted for
inventions based on acuity level and potential for impact. The program
provides information and resources to assist in reducing or eliminating
risk factors, maintaining a healthy lifestyle, effectively managing their
condition and receiving the most clinically-appropriate, cost-effective
and timely diagnostic testing and procedures.
Diabetes Management Program
The Diabetes Management program seeks to reduce unnecessary hospitalizations
and health care costs and works to improve the health of those individuals
who have diabetes. The program provides information, interventions,
nurse coaches and other resources to help participants:
- Manage their condition through daily monitoring of key findings,
including weight and symptom changes, and symptom-specific management
- Adhere to physician treatment plans and drug therapy regimens
- Manage risk factors and co-morbidities
- Understand their condition and how to receive the most clinically
appropriate, cost-effective and timely diagnostics and procedures
Heart Failure Disease Management Program
The Heart Failure Disease Management program seeks to reduce unnecessary
hospitalizations and health care costs and works to improve the health
of those individuals who have heart failure. The program provides information,
interventions, nurse coaches and other resources to help participants:
- Manage their condition through daily monitoring of key findings,
including weight and symptom changes, and symptom-specific management
- Adhere to physician treatment plans and drug therapy regimens
- Manage risk factors and co-morbidities including hypertension, asthma,
diabetes, coronary artery disease, chronic obstructive pulmonary disease
and depression
- Understand their condition and how to receive the most clinically
appropriate, cost-effective and timely diagnostics and procedures
